Annual Exclusive Plan (AEP)
Includes but not limited to:
- Up to 10 clinical office visits (routine, urgent, tele-med, physicals, etc.) *, **, ***
- ALL in-office administered medications
- Lab testing ordered through our office (ordered by an OH provider)
- Free access to the secure patient portal to see results from anywhere on any device
- In-office skilled services (IV therapy, simple surgical procedures, stitches, splinting, etc.)
Flu testing, blood work, etc. is ALL included at no additional cost to you!
This option removes the worry of meeting deductibles linked to office visits, lab testing, and more. It can potentially save up to 95% off traditional insurance-billed costs!
** AEP expires 1 year from the date of purchase
*** Additional visits through the end of the contracted term are billed at the current self-pay rate.
* Cost of prescribed medications filled at local pharmacy are not included in AEP.
$1,000 annual fee. 50% due at signup/50% due at second visit.

Men's Health Annual Exclusive Plan
Includes but not limited to:

- In-office skilled services (IV therapy, simple surgical procedures, stitches, splinting, etc.)
This plan includes everything from our Annual Exclusive Plan but is ideal for men utilizing TRT. It includes all recommended labs required for this service as well as therapeutic phlebotomy and other services as needed.
